Horse Power for Life is currently looking for cancer patients/students who would be eligible to participate in this program. If you know someone who would qualify, please let us know or have him/her contact us directly.

Students must be well enough physically to participate in the program.

They must fill out a Student Registration Form which includes:

• Personal Profile
• Physician Consent Form which must be signed by your current physician and includes a list of allergies and current medications with side effects of each drug.
• Signed emergency medical treatment consent form.
• Students must sign both Horse Power for Life and Banbury Cross, Ltd. Forms (must be signed by a parent if under 18-yrs-old)

NOTE: This program is FREE to the cancer patient and one family member. If a family member will be participating, he/she will need to also fill out ALL forms listed above, except for the physician consent which is geared specifically to the cancer patient.
